Procedure to issue documents
- Ambulatory care Ask for a nurse in outpatient clinic

- Receive the documents after paying the document fees to the hospitalization administration.
- Hospitalization Ask for a nurse room of the ward at hospitalization ward

- Receive the documents after paying the document fees to the hospitalization administration.
If you want to read contents of medical records of the related legislations (Prohibition on leakage of secrets in Article 19 of the Medical Service Act, Reading records in Article 21 of the Medical Service Act, Article 13 of the Implementing Regulations in the Medical Service Act) or issue the copies, you can read them and issue the copies according to the specified laws and rules.
Informtion on preparatory documents
If a patient wants to read the contents of his/her medical records and issue the copies.
- Identification card with his/her own photo
If the patient’s lineal family member wants to read the contents of his/her medical records and issue the copies.
- The applicant’s identification card
- The patient’s own identification card
- The patient’s written consent in his/her own handwriting
- The patient’s own certificate of family relations and resident registration
- The patient’s power of attorney in his/her own handwriting
Application for reading the contents of his/her medical records or issue the copies by proxy

- If the patient is a minor, his/her certificate of family relations and resident registration
Medical certificates for soldiers
- The certificates can be issued only when you directly visit the hospital.
- Visit the hospital with the identification card and two 3×4 photos
- If they are reissued, please prepare the same number of your photos with the number of reissued copies
Death certificates
- The applicant’s identification with his/her photo, the deceased’s address in the resident registration place
- Visit the hospital with the documents that the applicant proves to have family relations with the deceased
- If they are reissued, the applicant should be the deceased’s lineal family member and Family Census Register ( Removed ) should be prepared.
