Medical certificates for soldiers

• The certificates can be issued only when you directly visit the hospital.
• Visit the hospital with the identification card and two 3×4 photos
• If they are reissued, please prepare the same number of your photos with the number of reissued copies

Death certificates

• The applicant’s identification with his/her photo, the deceased’s address in the resident registration place
• Visit the hospital with the documents that the applicant proves to have family relations with the deceased
• If they are reissued, the applicant should be the deceased’s lineal family member and Family Census Register ( Removed ) should be prepared.
